Your health insurance company might need you to supply certain information directly. It is your sole responsibility to comply with their request.

As a courtesy, we will file all insurance claims at Westfield Urgent Care. However, please provide all insurance information and a completed claim form at the time of service. Also understand that your health insurance policy is a contract between you and your company. We are not a party to that contract, and therefore, the balance is your responsibility whether your insurance company pays or not. Should you elect to file your own insurance claims, you will be responsible for all charges at the time of service.
